Declare _PyErr_GetTopmostException() with PyAPI_FUNC() to properly
export the function in the C API. The function remains private
("_Py") prefix.

The Py_FatalError() function is replaced with a macro which logs
automatically the name of the current function, unless the
Py_LIMITED_API macro is defined.

Changes:

* Add _Py_FatalErrorFunc() function.
* Remove the function name from the message of Py_FatalError() calls
  which included the function name.
* Update tests.</content>

This adds a new function named _PyErr_GetExcInfo() that is a variation of the
original PyErr_GetExcInfo() taking a PyThreadState as its first argument.
That function allows to retrieve the exceptions information of any Python
thread -- not only the current one.</content>
