<title>format-patch: show 0/1 and 1/1 for singleton patch with cover letter</title>

Change the default behavior of git-format-patch to generate numbered
sequence of 0/1 and 1/1 when generating both a cover-letter and a single
patch. This standardizes the cover letter to have 0/N which helps
distinguish the cover letter from the patch itself. Since the behavior
is easily changed via configuration as well as the use of -n and -N this
should be acceptable default behavior.

Add tests for the new default behavior.

Breaks in a test assertion's &amp;&amp; chain can potentially hide
failures from earlier commands in the chain.

Commands intended to fail should be marked with !, test_must_fail, or
test_might_fail. The examples in this patch do not require that.

Let a command-line --keep-subject (-k) override a config-specified
format.numbered (--numbered (-n)), rather than provoking the

<title>format-patch: autonumber by default</title>

format-patch is most commonly used for multiple patches at once when
sending a patchset, in which case we want to number the patches; on
the other hand, single patches are not usually expected to be
numbered.

In other words, the typical behavior expected from format-patch is the
one obtained by enabling autonumber, so we set it to be the default.

Users that want to disable numbering for a particular patchset can do
so with the existing -N command-line switch. Users that want to
change the default behavior can use the format.numbering config key.

Just because there wasn't a test for --numbered isn't a good reason
not to test format.numbered. So now we test both.
