Rename size() to encodedSize() for encoded types to allow std collection interfaces for types like FieldTable and Array.

The extra.xml fragment adds class defs for connection in session that are in line with latest spec but use old schema.
The preview codepath (99-0) remains unaltered.

 - Uniform holder for all body types, replaces MethodHolder.
 - Uses in_place constructors to avoid avoid body copy.

framing::AMQFrame:
 - Holds body in heap-allocated intrusive_ptr
 - Uses in_place constructors to avoid avoid body copy.

Removed/downgraded to TODO many redundant FIXME comments.
