Changes to connection lifecycle methods and Connection parameters:

 - Connection.open -> Connection.establish
 - Connection.connect() split into Connection.open(), Connection.attach()
 - Connection.disconnect() -> Connection.detach()
 - reconnect_hosts -> reconnect_urls
 - transport now takes tcp, ssl, and tcp+tls
