Fixed regression in the ``sqlalchemy.ext.instrumentation`` extension that
prevented instrumentation disposal from working completely. This fix
includes both a 1.4 regression fix as well as a fix for a related issue
that existed in 1.3 also.   As part of this change, the
:class:`sqlalchemy.ext.instrumentation.InstrumentationManager` class now
has a new method ``unregister()``, which replaces the previous method
``dispose()``, which was not called as of version 1.4.

I7eda77fed3d8e73df84b3651fd6cfcfe858d4dc9,  this set of changes
resolves all remaining flake8 conditions for those codes
we have enabled in setup.cfg.

Included are resolutions for all remaining flake8 issues
including shadowed builtins, long lines, import order, unused
imports, duplicate imports, and docstring issues.

This is a straight reformat run using black as is, with no edits
applied at all.

The black run will format code consistently, however in
some cases that are prevalent in SQLAlchemy code it produces
too-long lines.   The too-long lines will be resolved in the
following commit that will resolve all remaining flake8 issues
including shadowed builtins, long lines, import order, unused
imports, duplicate imports, and docstring issues.

expected
- clean up test_extendedattr.py and fix it to no longer leak
itself outside by ensuring _reinstall_default_lookups is always called,
part of #3408
- Fixed bug where when using extended attribute instrumentation system,
the correct exception would not be raised when :func:`.class_mapper`
were called with an invalid input that also happened to not
be weak referencable, such as an integer.

- rework the event system so that event modules load after their
targets, dependencies are reversed
- create an improved strategy lookup system for the ORM
- rework the ORM to have very few import cycles
- move out "importlater" to just util.dependency
- other tricks to cross-populate modules in as clear a way as possible
