simplification around various node list functions
- cluster_nodes/1 accepts 'ram', eliminating cluster_ram_nodes/0
- introduce is_only_clustered_disc_node/0 helper and get rid of
is_disc_and_clustered/0 and is_only_disc_node/0 in the process
Also inline disconnect_nodes/1.
