`Rack::Builder#parse_file` is essentially two things rolled into one. When
loading files that extend in `.rb`, `require` is used and there is no way
to provide the options.

Tidy up `Builder#initialize`.

These variables generally come too late to be useful. Removed `Rack::Lock`
which depends on these variables.

Decrease default allowed parameter recursion level from 100 to 32

Make Rack::Request#{[],[]=} warn even in non-verbose mode

Additionally, use the :uplevel keyword to warn, so it includes
the caller information in the warning output (similar to rb_warn).
This only works on Ruby 2.5+, so output will look a little odd on
Ruby 2.3/2.4, but I don't think it warrants separate handling in
older versions.
