MAINT: Fixup release notes, changelogs after #9238 merge.

The merge readded the removed changelog information to the
1.12.0-notes.rst file, together with a couple of fixes. Remove the
changelog information again and apply the fixes to the changelog. Also
make some small fixes to the other changelogs by adding a Changelog
title.
