Preparation for session thread safety overhaul:

 - simplified SessionState, responsibility for protocol states now in Handlers
 - qpid::RefCounted, qpid::intrusive_ptr reference counting support.
 - build boost unit tests as single exe, speeds up testing.
 - fixed leak in AsynchIOAcceptor.cpp
